Our key services

We are licensed & insured to provide support that is person centered around individual goals and needs. We work in collaboration with families & their support coordinator to achieve these goals. 

Day Support (Group & Individual

Our day program is Monday to Friday with some weekends. We service people in the Weber, Davis & Salt Lake Counties. We do many community based activities that are age appropriate and allow our people to experience daily life just the same as their neurotypical peers. We keep our groups smaller to maintain a calmer setting with more one on one attention between individuals and staff.

Supported Living in Home

We assist individuals in maintaining independence and safety at home by building daily living skills such as meal planning, budgeting, medication reminders, household management, transportation, and social skills—for example, supporting a client with grocery shopping, preparing meals, managing their household, and traveling to work or community activities.

Transportation

We can provide you with reliable transportation to and from your home for day support programs. We do not use buses or large vans & no company marked vehicles. This allows for low stress transportation. Unfortunately, at this time we cannot provide wheelchair accessible vehicles. 

Host Home Support

Our Host Home program offers a unique shared-living experience for adults with intellectual or developmental disabilities. By moving away from institutional settings and into a private, nurturing home, individuals receive personalized 24/7 care and the opportunity to build meaningful social connections. We focus on "life-sharing," where our clients become part of a stable family environment—joining in traditions, community outings, and daily routines that promote personal growth and autonomy.

Our key service codes

We provide a range of services tailored to individual needs, operating under the following common service codes:

  • Day Support
    • Group (DSG): Structured community-based activities in a group setting to prevent isolation and develop life or vocational skills.
    • Individual (DSI): One-on-one daytime habilitation and support, used when a group setting is not available or appropriate.
  • Supported Living (SL1, SLH, SLN): Assistance for individuals living in their own homes to maintain health and safety. It covers personal care, household tasks, and community access.
  • Companion (COM, CO1): One-to-one non-medical care and supervision. Includes assistance with meal prep, laundry, and access to social experiences.

 

 

  • Respite (RP1–RP8, RPS): Provides temporary relief for the primary caregiver. Codes vary based on whether it is "Routine" (RP2), "Exceptional Care" (RP3), or includes "Room and Board" (RP4, RP6).
  • Homemaker (HSQ, HS1): General household tasks such as cleaning, meal preparation, and laundry to help maintain a safe living environment.
  • Personal Assistance (PAC, PA1, PA2, PA3): Support with activities of daily living (ADLs) like eating, bathing, and dressing for individuals medically capable of directing their own care.
  • Transportation (MTP, DTP): Financial assistance or payments for motor transportation to access community services and activities.
  • Host Homes (HHS): Out-of-home placement where an individual lives in the private residence of a trained "professional parent" or family.
  • Support Employee (SEC, SED, SEI): Formerly referred to as "Supported Employment."
    • Individual (SEI): One-on-one coaching to help an individual find and maintain a job.
    • Group (SED): Support provided in a group work setting to gain experience and job-readiness skills.
  • Personal Budget Assistance (PBA): Support for individuals needing help managing their personal finances, bills, and financial needs.
